Application
Processing Procedure: We will not be able to process your application until we have received your application fee, which should be sent along with your application. Simply print out the attached .PDF and fill out the application in a legible manner. Once complete and you have organized for us to receive two recommendations, mail or fax the application and mail the application fee. Those whom who request a recommendation are responsible for mailing their information to us.
*If we have not contacted you 2 weeks after sending your application please contact a SLAM representative ASAP.

- April 7 – $800
- May 5 – $750
- June 9 – $500
Total: $3,250.00
*You will not be confirmed a spot on Da’ Mission until your application is accepted and your deposit is paid.
If application and deposit is received by:
- March 10 – Total: $3,250
- After March 10 – Total: $3,450 or more*
*Applications are accepted up to June 1st. Participant is subject to any additional fees that may be associated with late sign up including but not limited to airline fees, train ticket fees, and accommodations. Request application by contacting us at the address below.

Preparation Information
Passport: All participants must obtain a passport at least 10 weeks prior to trip (by May 1, 2010). This is of utmost importance; it can take up to 8 weeks to attain a passport. If you need to renew your passport it can be done online (click here – Passport Renewal).
Visa: Each participant is responsible for filling out the application to receive a India Tourist Visa. This must be done no earlier than April 1st and no later than June 1st.

Immunization Requirements: Be up to date on all childhood immunizations. In addition the following suggested immunizations coincide with current guidelines set out by CDC (Center for Disease Control) for this area but are not required. Please discuss your travel plans and personal health with a health-care provider to determine which vaccines you will need.
